This is a reminder that the due date for the Application for State Library Aid / 2008NJSL-10APP is fast approaching. The application is included in the 2008 Public Library Survey as a hyperlink in between PART I and PART II of the survey: http://collect.informata.com/.

As an added convenience I have attached copies of the Application and the instructions in this email.

Please complete and return to my office an original, signed copy of 2008NJSL-10APP. If an amount is entered in Line 4, include an itemized statement detailing the additional expenditures. This statement must be signed by the CFO of the municipality/county, an authorized member of the board of trustees/commission and the library director. The application must be postmarked no later than March 15, 2009. The application must include the preparer's certificate number to be complete.
