This is an important reminder that the due date for submission of Public Library Survey 2009 / PCSA form is April 2nd, 2010. It is important to login into your respective library surveys and start filling the general information to ensure that the survey is functioning.

The survey can be accessed at http://collect.btol.com/. Login information is the same as previous year. In case you do not have the login information, contact me by email or phone. All instructions are included in the survey and for easier access to instructions while in the survey, you may click on the hyperlinked item number. A pop-up window will provide instruction for the respective item number.

Attached is the list of libraries that have not yet logged into the survey. To avoid the last minute rush it is recommended that the completion of survey process be started as soon as possible. Please note that filling out all the required information takes quite some time.

Accuracy Certification (The last item in the Survey Navigation) is to be mailed after completion of the survey.

Important note: There will be no extensions given for survey submission. IMLS is requesting timely submission of the survey so they can prepare national reports on time.
