Based on your questions, I\x92ve updated the form to make it easier and provided a sample. You should be able to enter your own numbers and see how things work out. This is just a draft as there may need to be some changes made, but I wanted to get this to you before the weekend. Please note that no formula has been developed yet for Line 16 and this line will have to be checked manually.
