Dear Colleagues,

The Data Year 2014 Annual Survey will open on Monday, January 12, 2015.  A reminder email with your library’s usernames and passwords, as well as the URL for the survey, will be sent prior to that date to the person responsible for the survey at your library.  If the person responsible for the survey at your library does not receive a reminder email please email me.

The deadline for survey completion by statute is March 1, 2015.  However, the State Librarian has granted everyone a two week extension.  The final deadline for the survey is March 15, 2015. 

There is one additional data element this year, the name and definition of which can be found in the instructions for the survey which can be found at http://www.njstatelib.org/wp-content/uploads/2014/09/FY16-INSTRUCTIONS.pdf             

All of the forms for the survey (Application, Accuracy Certification, etc.) can be found either at http://www.njstatelib.org/services_for_libraries/resources/per_capita_state_aid/ under the Data Year 2014 heading or on the survey website after login.
