Summer Reading 2015 Survey Open

We are using Google Forms for you to report your summer reading statistics for 2015 this year instead of SurveyMonkey. Don't forget to keep a copy of your statistics for your own records!

Our final report of all the libraries' statistics requires only the total number of books read. If your library keeps track of reading using something different, such as hours or minutes, please convert them to numbers of books using this formula: 1 book = 1 hour (or 60 minutes), 1 book = 100 pages.

Please Note: You will not receive summer reading materials for the 2016 program without submitting statistics for 2015.

The deadline to submit your statistics is October 5, 2015. This is a FIRM deadline because the final report from the statewide program is due at the end of October.
