The Data Year 2015 Annual Survey is now closed. Thank you all very much for your cooperation in filling out the survey and submitting your accuracy certifications. For those who were unable to submit their survey by the filing deadline
an email has been sent to the person who is responsible for your survey with a link to a form to request an exception to the filing deadline. Instructions for completion of the form are included in that email. Once again I would like to thank everyone for
completing the survey and if you have any questions about it please let me know.
